Understanding Inlays and Onlays: The Middle Ground Between Fillings and Crowns

When it comes to restoring a tooth that’s been damaged, most people assume they have only two options: a simple filling or a full dental crown. But what if there was a solution that falls right in the middle? Enter inlays and onlays—the lesser-known yet highly effective restorative dental treatments that can protect your teeth while preserving more of their natural structure.

At Awesome Dentistry in Oakbrook Terrace, IL, we believe in empowering our patients with knowledge about their dental health. If you’ve been told you need a filling or crown but want to explore all available options, this guide will break down everything you need to know about inlays and onlays.

What Are Inlays and Onlays?

Think of inlays and onlays as precision-crafted puzzle pieces designed to restore a damaged tooth while blending in seamlessly with your natural smile. They are custom-made restorations that fit inside or over a tooth, providing a solution for damage that’s too severe for a filling but doesn’t require a full crown.

Inlays sit inside the tooth, filling the space between the cusps (the raised points of a molar). Onlays extend over one or more cusps, covering a larger portion of the tooth’s surface. Essentially, they act as a conservative alternative to crowns by preserving as much of the healthy tooth as possible while still providing durable, long-lasting protection.

When Are Inlays and Onlays Recommended?

You might be wondering, why would I need an inlay or onlay instead of a standard filling or crown? These restorations are ideal for patients who have moderate tooth decay that is too extensive for a traditional filling but does not require a full crown, have cracked or fractured teeth that need reinforcement without complete coverage, need to replace old, failing fillings with a more durable and aesthetically pleasing solution, or prefer a stronger, longer-lasting option compared to composite fillings.

If you’ve ever had a large filling that eventually broke or wore down, you’ll understand why inlays and onlays can be a smarter choice—they offer superior strength and durability without completely reshaping your tooth like a crown would.

What’s the Difference Between Inlays, Onlays, Fillings, and Crowns?

To put things into perspective, traditional fillings are best for minor decay and small cavities inside the tooth. They offer moderate durability, lasting around five to ten years, and require minimal removal of the tooth’s structure. Inlays, on the other hand, fit inside the cusps of the tooth and are designed for larger cavities. They are significantly more durable, lasting ten to thirty years, and preserve more of the natural tooth compared to a crown.

Onlays cover one or more cusps of the tooth and are ideal for fractured or heavily decayed teeth. Like inlays, they can last ten to thirty years but remove slightly more tooth structure. Crowns, in contrast, cover the entire tooth and are best for severe decay or damage. They are the most durable, lasting fifteen to thirty years or more, but require more of the tooth to be removed before placement.

The Process: What to Expect When Getting an Inlay or Onlay

One of the biggest concerns patients have before any dental procedure is what the process will be like. The good news? Getting an inlay or onlay is straightforward and relatively painless.

Step 1: Diagnosis and Preparation

After a thorough examination at Awesome Dentistry, we’ll determine if an inlay or onlay is the best treatment option for your tooth. If so, we’ll carefully remove the damaged or decayed portion of the tooth while keeping as much of the healthy structure intact as possible.

Step 2: Impression and Temporary Restoration

Once the tooth is prepped, we’ll take a digital scan or traditional impression to create your custom inlay or onlay. Since these restorations are custom-made in a dental lab, you may receive a temporary filling or onlay to protect the tooth while your permanent restoration is being fabricated.

Step 3: Placement and Bonding

In about one to two weeks, your inlay or onlay will be ready. We’ll ensure a precise fit before permanently bonding it to your tooth with a strong dental adhesive. Once in place, it will be polished to blend seamlessly with your natural enamel.

Why Choose Inlays and Onlays Over Traditional Fillings?

While traditional fillings are widely used, they come with some downsides—particularly for large cavities. Over time, fillings can shrink, crack, or weaken the surrounding tooth structure, leading to additional decay or fractures. Inlays and onlays, on the other hand, offer several distinct advantages.

They are stronger and more durable, made from materials like porcelain or composite resin, and withstand chewing forces better than traditional fillings. Their custom-made nature ensures a perfect fit, reducing the risk of bacteria and decay creeping back in. Unlike metal fillings, these restorations match the color and translucency of your natural teeth, making them more aesthetically pleasing. Additionally, while fillings may need replacing every five to ten years, inlays and onlays can last decades with proper care.

Caring for Your Inlays and Onlays

Just like natural teeth, inlays and onlays require good oral hygiene and regular dental visits to stay in top condition. Follow these simple tips to keep your restoration in excellent shape. Brush and floss daily to prevent plaque buildup around the edges of the restoration. Avoid chewing hard foods like ice or hard candies that could potentially chip the restoration. Schedule routine checkups at Awesome Dentistry to monitor the health of your inlay or onlay. If you grind your teeth at night, wear a nightguard to protect against excessive wear.

With the right care, inlays and onlays can provide a lifetime of reliable function while keeping your smile healthy and beautiful.

Is an Inlay or Onlay Right for You?

If you’ve been told you need a filling or crown but aren’t sure what’s best for your tooth, consider an inlay or onlay as an alternative. These restorations offer a perfect balance of strength, aesthetics, and conservation of your natural tooth structure.
